This form is for Pups Up products that have been determined to have a manufacturing defect during normal, intended use.
The following are not covered under our warranty:
Cosmetic fading or flaking of coated hardware finishes
Patina, tarnishing, or staining with brass hardware
Staining or discoloration of Biothane® webbing
Damage caused by chewing, biting, dragging, or other misuse
Cuts, tears, or other damage to the Biothane® webbing

Photos are required. Claims submitted without clear photos of the product will not be reviewed.
Proof of purchase is required. Acceptable proof includes an order number, receipt, bank statement, or other documentation showing your purchase.
Failure to meet the above requirements may result in your warranty claim being denied.
